Innovation

Since our founding, we have been an innovator in providing medical house calls. We have built a collaborative, evidence-based care model that has shown improvements in clinical and financial outcomes for patients and senior living communities. We are one of only two medical house call practices in Ohio chosen by the Center for Medicare and Medicaid Innovation for the Primary Care First program. Over 80% of our patients are in the highest complex risk tier of the CPC+ Track 2 program, meaning that we care for the highest risk people in society. We will continue to focus on innovation, leaning into new Medicare programs such as Primary Care First and Seriously Ill Population care models, while providing compassionate and quality care to those we serve today.
