Western Reserve Medical Group patient portal is now available! Patients and authorized patient representatives may access clinical information. The portal will give the representative access to and the ability to view, download and/or transmit the patient’s ENTIRE health information records kept within the electronic medical records.
To request access, please contact your scheduler at 1-800-807-6555.
Please be sure to have the patient’s social security number available. This is required information to create the portal account.
Our staff will create the user profile, and send a registration email (sample email below). The patient representative must click the hyperlink in the email (noted in the example below in the red box and referenced by the red arrow) to create the portal account.

gEHRiMed software is unable to limit access to certain health information, including but not limited to mental health information (CGS 52-146(D), substances abuse treatment information (42 CFR 2.1-2.67), and HIV/AIDS- related information (CGS 19A-585(A). Patient acknowledges and agrees that if they choose to have certain health history limited, no patient portal account should be created for that patient. Additionally, if a patient portal account is created, and the patient subsequently chooses to have certain health history limited, Patient understands that they must submit a request to remove access immediately to close the patient portal account
Western Reserve Medical Group will remove access for the patient representative if the patient at any time revokes consent and/or authorization for a designated patient representative and/or an authorization or consent for a designated patient representative expires.
