William (Bill) Mills, M.D. is Senior Vice President of Medical Affairs at BrightSpring Health Services. BrightSpring is the leading provider of comprehensive home and community-based health and medication management services to complex populations in need of specialized care. At BrightSpring, Dr. Mills is focused on furthering the organization’s value proposition by leading its outcome measure development, reporting, and publishing. This work has led to seven peer-reviewed publications and numerous symposia presentations. Current active studies include: the development of value-based outcome measures in the field of intellectual and developmental disability (IDD); variation in hospitalization rates among senior living communities; days spent at home as an outcome measure in community-based care settings; the consultant pharmacist’s impact on acute care avoidance; and the effect of a highly coordinated COVID-19 pandemic outbreak response in a diversified, national healthcare delivery system. Dr. Mills has served as Chair of the organization’s Outbreak Committee throughout the COVID-19 pandemic with a primary charge of protecting 55,000 employees and 350,000 patients and clients that the organization touches daily. The outbreak response has been highly successful, achieving an infection rate of one-half of that of the U.S. population, and resulting in several peer-reviewed studies that have been referenced by many academic, industry and public health outlets, including the World Health Organization. Dr. Mills is a board-certified physician who has spent his career providing home-centered care to chronically and seriously ill patients, and he has personally made over 21,000 medical house calls in his career.

Dr. Miranda M. Huffman, M.D.
Medical Director
Dr. Huffman is a family physician in Nashville, Tennessee. She is a graduate of the Northeast Ohio Medical University (NEOMED). During medical school, she took a leave of absence to pursue a Master’s in Education at Kent State University and a fellowship in medical education through NEOMED. She went on to complete her family medicine residency at the University of Michigan. She has served as family medicine faculty at the University of Missouri – Kansas City and Meharry Medical College in Nashville, Tennessee, prior to joining Western Reserve Medical Group in 2020. She continues to serve as adjunct clinical faculty at Meharry Medical College, where she mentors’ students in a free clinic and teaches clinical medicine. Dr. Huffman loves the variety and long-term relationships that come with being a family physician. Her approach to patient care focuses on understanding her patients beyond just their list of diagnoses. She believes that medicine is a partnership between the physician and the patient to achieve mutually agreed-upon goals. Dr. Huffman thinks we are all capable of growth with the right support from the right team. She is certified as an HIV specialist and has a particular interest in the care of patients with substance use disorders and chronic pain. She lives with her husband, Jeremy, daughter, Betty, and pug, Koop. She enjoys cooking, reading, spending time in nature, and indoor cycling.

Dr. Richard Li joined Western Reserve Medical Group in 2020 bringing 19 years of clinical experience. He graduated from Southeast University medical college, completed his resident training in Summa health system internal medicine resident program in Akron, Ohio. Earning lots of complicated clinical experience including hospitalist, wound care, home health, geriatric for his quality and patient-centered care. Dr. Li’s passion is driven by the bonds he builds with his patients over the course of their lives and enjoyment he holds in keeping them healthy and well. In his free time, Dr. Li can be found playing tennis with friends, hiking in state forest park and world travel.

David L. Anstadt, MD
Dave Anstadt is a board-certified Internal Medicine Physician who had a large practice for over 30 years in in Warren Ohio. In April of 2015, he joined Western Reserve Medical Group and since that time, has enjoyed treating our amazing seniors. He completed a residency in preventive medicine at The University of Cincinnati as well as a residency in Internal Medicine. This knowledge of prevention has allowed him to focus on preventing diseases as well as treating them. His love for seniors started when he grew up on a farm in central Pennsylvania. His grandparents were very involved in hid life and he met a lot of seniors on his paper route. His dad was a WWII Marine, so he always enjoys hearing the stories from the veterans he takes care of. An Eagle Scout, graduated with distinction from Penn State to then graduated from Temple University Medical School in Philadelphia followed by his internship at Mercy Hospital in Pittsburgh. Dr. Anstadt has two wonderful children, plenty of pets and memorable vacations add balance to his life. Please call him Dave. He is truly interested in you as a person as well as being committed to keeping you healthy.

Dr. Larita Frazier O'Bannon

Dr. Larita Frazier O’Bannon is a graduate of the University of Cincinnati College of Medicine. She completed her residency with St. Elizabeth Medical Center and is board certified in Family Medicine. She has been practicing medicine for the past 29 years, including practices in women’s health as well as geriatric medicine. She has also served as the Medical Director of Phoenix International Life Sciences, overseeing complex clinical trials. In addition to her medical practices, she has been involved with hospice care functioning as a Medical Director for hospices within the Greater Cincinnati/Dayton area. Over the past 15 years, Dr. Frazier O’Bannon has been engaged in a house call practice providing care to vulnerable geriatric patients and intellectually and developmentally disabled individuals. Dr. Frazier is passionate in her care for individuals with complex medical conditions and individuals with special needs and developmental challenges. In her free time, Dr Frazier O’Bannon enjoys traveling, especially taking leisurely Caribbean cruises.

Dr. Alexis Parks Myton

Dr. Alexis Parks Myton is an Internal Medicine Specialist in Dayton, OH and has 30 years’ experience in the medical field. She graduated from Wright State University Boonshoft School of Medicine in Fairborn, OH and then completed a residency at Miami Valley Hospital's Internal Medicine/Primary Care Residency program which was an affiliated hospital and the Dayton region's premier Level 1 Trauma hospital. While in private practice, Dr. Myton had past assignments as Assistant Clinical Professor for the Wright State University Boonshoft School of Medicine, served on the Kaplan College (formerly OIP&T) Advisory Committee for the Medical Assistant program, and precepted many medical residents and students in the hospital clinics and medical-surgical units where she was on staff as an attending physician. She also has several years of experience working in Urgent Care centers as well. Being the child of a retired military officer, she was proud to be able to provide medical care during her residency to veterans at the Dayton VA medical center and later in her career to the retirees and their spouses in the Internal Medicine Clinics at Wright-Patterson Air Force Base in Ohio. The fact that she had grandparents and great grandparents that lived into their 90's helped fuel her passion for caring for our nation's aging population. Dr. Myton has been married to her husband for 30 years and they have raised 3 children together.

Omar Diaz, MSN, CNP

Omar R. Diaz joined Western Reserve Medical Group in August 2018, bringing 4 years of clinical experience. He is a nurse practitioner with RN experience in hospital environments, rehabilitation and long-term care. He received his Bachelor of Science in nursing from the University of Toledo. While working in a busy medical intensive care unit, Omar completed his Master of Science in nursing with a subspecialty in adult and geriatric care from the University of Akron. He is a board-certified adult/gerontology nurse practitioner and licensed in Ohio. His focus is on providing holistic, compassionate care while ensuring comprehensive evaluation and management of adult and geriatric patients, including care of those with complex medical and psychosocial needs in all settings. Omar grew up in New Jersey and later moved to Ohio to pursue his schooling. His passion for learning and practicing the art of nursing is reflected in his overall compassionate care: “I believe that every one of my patients should receive the type of compassionate care that I would want for one of my own family members and communication with the patient’s family is the key to good healthcare.” Omar lives in North Ridgeville, OH. He is a dog lover and loves to travel and explore new places.

Alison Dubasak, Family-CNP

Alison joined Western Reserve Medical Group in 2019 bringing with her over 12 years of clinical experience. She has been a Board-Certified Nurse Practitioner since graduating Chamberlain College of Nursing Master’s Program in 2018. She has an extensive background in wound care, minor surgical procedures, emergency medicine, cardiovascular, pulmonary, gastrointestinal and endocrine diseases. She also has experience in teaching as having served as an adjunct facility member for nursing students at Kent State University. She has certifications in wound care and hyperbaric oxygen therapy. Experienced in management of chronic and acute conditions, medication and disease education, injections, minor procedures, and overall health promotion of the geriatric population. Treating our older population with a holistic approach and allowing them to age with needs met in a dignified, respectful manner. When not working, Alison enjoys spending time with her family on the boat in Lake Erie. She also enjoys traveling and camping as well.

Laura is a member of The American Academy of Home Care Medicine, and The American Academy of Nurse Practitioners. Laura has been practicing since 2009 and has been working in medical house calls since 2011 which has allowed her to develop a stronger partnership with her patients while providing them with quality care. In her free time, Laura enjoys riding horses and camping with friends and family.

Stacie is a Board-Certified Family Nurse Practitioner with the American Academy of Nurse Practitioners and is licensed to practice in the state of Ohio. She moved to NE Ohio from Florida where she worked in medical marketing and nonprofit development, serving some our countries most fragile populations for more than 20 years. Wanting to do more Stacie went back to school receiving her BSN and MSN, FNP from Chamberlain University School of Nursing in Chicago, Illinois. Stacie began her nursing career at The Cleveland Clinic, Medical ICU as an RN under the renowned Pulmonary Institute. Stacie’s passion for caring was developed at a young age growing up in a multi-generational home, witnessing home care and hospice management for both of her grandmothers and her father with early onset dementia. Experiences that drive her desire to provide compassionate medical care today. Stacie and her husband, Andy are the proud parents of three daughters and a son. Graced with beautiful grandchildren. Living on a 220-year-old legacy farm in NE Ohio. She enjoys her family, trail riding and motorcycle touring.

Keiko is a dual family and psychiatric mental health practitioner. She graduated California State University Northridge with a Bachelor of Science in Kinesiology in 2003. She attended Xavier University and completed her Master of Nursing in 2010 then earned her Post Master Certificate of Family Nurse Practitioner in 2014 and obtained her national certificate through the American Academy of Nurse Practitioners. She also completed her Post Master Certificate of Psychiatric Mental Health Nurse Practitioner in 2021 and obtained her national certificate through the American Nurses Credentialing Center. She has an excellent work ethic, positive energy and respect, and is passionate about providing care to the senior population. In her free time, Keiko enjoys traveling and spending time with her husband and son.

Mary Thiel, ACNP-BC

Mary Thiel APRN joined Western Reserve Medical Group in 2021. She has been a nurse for 35 years, having graduated from the University of Cincinnati in 1986 with her bachelor’s degree in nursing. She then joined the United States Air Force on active duty as an operating room nurse where she remained until 1991. She returned to Cincinnati to continue as an OR nurse, then an office manager for a medical practice, until she enrolled in graduate school at the University of Cincinnati in 2003, graduating in 2005 with her master’s degree in Adult Acute Care Nursing. She is board certified. She remained in the Air Force Reserves and retired after 20 years in 2007. For the past 15 years she has practiced as an adult nurse practitioner in long-term care and primary care settings, with a preference for the care of the elderly and disabled populations. Elder and disabled adults are unique from their younger adult counterparts. There is often extensive family involvement, requiring a more holistic approach.

Deborah has over forty years’ experience as a registered nurse with certification in Advance Practice Family Care. A range of experience from pediatric critical, infusion therapy, urgent care to primary care. Belief that education is a base to providing quality care. Educated at Central Ohio Technical College, Ohio University, Ohio State University and Post Graduate at Otterbein University. Deborah is passionate about helping her patients and providing the best care possible while keeping them safe in their home. Besides enjoying providing holistic primary care to home bound patients she enjoys her family! Especially spending time with her granddaughters!

Rebecca is a nurse practitioner prepared in family medicine from Kent State University and she has also earned a BSN from The University of Akron. While completing formal education for her bachelors and masters, Rebecca concurrently worked at the bedside in a variety of care areas such as intensive care units, endoscopy unit and worked as adjunct faculty member for Kent, Akron and Tri-C. She remains employed by Kent State University School of Nursing, as an adjunct faculty member. Rebecca comes to Western Reserve Medical Group with several years of nurse practitioner experience focusing on preventive and proactive medicine. She lives in the Copley area with her husband and two children. On her free time, she enjoys running, yoga, cooking, vacationing, hiking in the Cuyahoga Valley National Park and spending time with family and friends.

Jessica Maag is an Adult-Geriatric Nurse Practitioner joining the Western Reserve Medical Group team in April 2022. A native of Canton OH, she is a 2013 Graduate of Malone University Bachelor of Science in Nursing program. Jessica relocated to South Carolina and has 7 additional years of RN experience in acute care settings. Jessica graduated with her Master of Nursing from Walden University in 2019. She practiced as an Adult Geriatric Nurse Practitioner in a family practice for 2 years before deciding to relocate back to Ohio. Her goal and passion as a provider are to establish the best quality of life possible. She strongly believes this can be achieved with a good relationship with her patients and their support system. It brings her joy as a provider to see and share with patients’ improvements in daily life and activities. Jessica and her husband Adam have two children, daughter Lilly, and a son, Casey. She enjoys running outside with her dog, Riley. Jessica enjoys boating in the summer and in the winter can be found on the ski slopes with her family.

Leonard Cooley, DNP, ARPN, FNP-BC
Leonard is originally from the Los Angeles, CA area. Having also spent time growing up in Arkansas, Leonard moved to Wisconsin in 1992. He obtained his ADN from Southwest Wisconsin Technical College in 2002. He began his nursing career working in a rural critical access hospital then switched direction, moving into long-term care and mental health. Realizing his niche, he returned to school in 2011, obtaining his BSN and BA Psychology from Clarke University, Dubuque, IA. In 2018 he obtained his DNP, also from Clarke University, and began practice in Palliative and Hospice medicine. He holds an Advanced Dementia Care Specialist certificate from the University of Wisconsin, Madison. Although board certified in family practice, Leonard has special interest in geriatrics and mental health. He also enjoys teaching mental health coursework to undergraduate nurses at Clarke University. He currently lives in Xenia, OH with wife, Tiffany. They have one daughter, Luvina, who is a junior at Ohio University, Athens. Leonard enjoys camping, fishing, and is an avid baseball fan...Go Cubbies!

She started her nursing career in 1997 as a Licensed Practical Nurse and has worked her way through the ranks, graduating in 2016 with her Master of Science in Nursing degree. Lori’s experience includes long term care, medical-surgical, telemetry, and primary care nursing. Nursing is a passion for Lori, as this has been her life goal for as long as she can remember. Lori is married to John and together they have three children Caitlin, Jacob, and Joshua. They also have three grandchildren Mason, Landyn, and Phoenix. Lori and her family call Cadiz, Ohio home.

Recently joined Western Reserve Medical Group in 2022, providing care for patients in the Kentuckiana area. JoAnn brings with her knowledge from over 20 years of diverse experience in the medical field. Starting as an EMT, her desire for a better understanding of the human body and its conditions as well as the desire to help those in need, have brought her to where she is today. Initially completing a degree in Emergency Medical Services in Florida, JoAnn then moved to Kentucky where she completed her bachelor’s and then Master’s Degree from Spalding University. Much of her nursing career has been spent working in Intensive Care Units caring for the critically ill or injured. For several years, she also worked as adjunct faculty for Bellarmine University as well as University of Louisville School of Nursing. Outside of work, JoAnn enjoys spending time with family as well as her four dogs!

She is a graduate of Huron School of Nursing, Slippery Rock University, and Troy University of Alabama. She is also a certified school nurse. She worked as an Athletic Trainer and then decided to pursue nursing to spend more time with her growing family. She has worked in ICU and as a school nurse. She has worked as a hospitalist as an FNP and also in Urgent Care. She decided she wanted to develop relationships with her patients and families like the one she has with her personal physician. She enjoys, running, kayaking, hiking, and spending time with her family.
